Rise Above Support Services LLC is contracted statewide to provide Attendant Care, Habilitation, and Respite.

We are very experienced in navigating the complex DDD system and can assist you in securing services needed.

Attendant Care

Attendant Care Services provide persons with disabilities the supports they need to maintain the highest quality of life. Based on the Individual Service Plan (ISP) we provide trained staff to help with lifting and transfers, bathing, shopping, cooking, and cleaning. If you prefer family members, neighbors, and friends can also all be referred to us, and we will provide them with the required trainings and certifications they will need to work. In fact, parents of persons with disabilities over the age of 18 can usually be certified to provide this service. Our staff meets all of the State of Arizona requirements including fingerprint clearance, CPR, First Aid, Article 9, background checks and the new Direct Care Worker (DCW) training.

Habilitation
Habilitation

Habilitation Services prepare people with disabilities to live more independently in the community through learning and developing new skills, abilities, and behaviors. Common areas for training include: self-care, communication, socialization, safety, physical and emotional expression, pre-vocational skills, leisure activities, and problem solving. The service is provided by a trained Direct Support Professional who works one-on-one with the client based on goals and outcomes determined by the individual and his or her team. Parents and guardians can often be trained to provide Habilitation to their adult children. We also provide more intensives type of Habilitation commonly called Hab M and Hab B for persons diagnosed with Autism or special behavioral challenges.

Respite

Everyone needs a break sometimes! Our Respite program is designed to let you relax with the knowledge that our trained staff will see to your child’s needs and comfort. Or, if you would prefer, we can arrange to train and certify someone you refer like a relative, neighbor, or person from your church to provide the care.
